Who is the supervisor & employer for IHSS Provider????

I'm applying for a job and they want to know my work history. I worked as an IHSS Provider couple years ago in San Francisco. I don't remember who was considered my supervisor and employer. Would it be the IHSS Recipient or the company, as in "California of Department of Social Services (CDSS), San Francisco Human Services Agency (SFHSA)." Whose telephone number and address do I put?
